When you submit this form, please include:

• the behavior contract (also available in the Parish Office)
• a COPY of the student's baptismal certificate
• a COPY of the student's First Reconciliation and First Communion Certificates
• If 1st year, the $225 registration fee which includes the $75 tution plus $150 for retreat.  If 2nd year and have completed retreat, fee is $75.

Documents can be turned in to the main office, the youth office, or the CCD office, or can be faxed to St. Gabriel's at (858)748-5764 ATTN: Youth Office.

Forms will not be accepted without all documents and fees.

All Forms must be submitted by August 20th, 2011.
